September 30, 2026 — TeamSense Forms is now available to customers. With Forms, HR and operations teams can build and send forms over text, without asking employees to download an app, create an account, or use company email.

Think about an open shift. A supervisor needs to find out who can cover it, but the request goes into a group text, and the replies have to be sorted out by hand. With Forms, the team can send an overtime sign-up to the employees who should receive it and review their responses in TeamSense.

Forms that know who they’re going to

Most form tools give you a link and a list of answers. TeamSense Forms uses the employee phone numbers, rosters, and supervisor relationships already in TeamSense. You can send a form to a specific group without building a separate contact list, and responses come back connected to employee information your team already uses.

Employees can also request a form themselves. With keyword access, someone can text a term such as OVERTIME to get the form they need.

Build it, send it, and follow up

Forms is self-service, so teams can create and update forms as their needs change. The builder supports custom questions, question ordering, and branching logic. Teams can start with a template or describe what they need in plain language and use the AI-powered builder to create a first draft.

Once a form is ready, TeamSense can translate it into the languages your organization has enabled. Access controls help keep forms available to the right employees or sites. When someone submits a response, notifications can go to an administrator, the employee’s manager, or an external email address. Teams can also see how many forms were sent, response rates, completion time, and aggregated answers. 

That opens up uses well beyond overtime. Teams can use Forms for open shift sign-ups, safety checks, incident reports, policy acknowledgments, new-hire check-ins, employee availability, and maintenance requests.
